HAZARD DETECTED!

😵

A hazard line was crossed! This leads to fatal or severe electrical injury.

The worker approached high-voltage equipment without following safety protocols, resulting in an electrical shock incident.

SAFETY PROTOCOL SOLUTION

😊

Proper safety steps to take BEFORE servicing high voltage equipment:

  1. De-energize the equipment and lockout/tagout (LOTO)
  2. Test for zero energy with a verified voltage tester
  3. Establish restricted approach boundaries
  4. Wear appropriate PPE (insulated gloves, face shield, etc.)
  5. Have a qualified observer present
  6. Follow company-specific safety procedures
